I give my team registered above permission to participate in the USA Youth Hoops event and hereby waive, release and forever discharge any and all claims against the organizers, sponsors, organizations or persons providing facilities, volunteers or any agent, and affiliate or employee thereof for damages and/or injuries to the participate which may arise from the participation in the USA Youth Hoops event.
