Thank you for registering for Ohio Players & SW Ohio Basketball Tournaments. Please remember that registration is not complete until full payment is received. There are NO REFUNDS or CREDITS. If you have any questions, please call Tom at 513-235-9007 or James at 513-289-5732 I certify that my son and/or daughter has had a physical examination by a licensed physician within the last year and is in sound physical condition for participation in Ohio Players Tournaments. I authorize the staff of Ohio Players Tournaments & SW Ohio Basketball to act for me to their best judgement in situations requiring first aid or medical attention. I understand participation in athletics has certain inherent risks and assume responsibility for those risks. By registering for this tournament, I confirm that I understand that payments are non-refundable, regardless of prior notice of cancellation or the reason for cancellation. I further waive and release Ohio Players Tournaments & SW Ohio Basketball as well as staff, employees, contractors & volunteers along with those directing these tournaments from any and all liability for any injury my child might incur while participating in any and all Ohio Players & SW Ohio Basketball Tournaments.
