If you have any questions, please call Tom at 513-235-9007 or James at 513-289-5732 I certify that my son and/or daughter has had a physical examination by a licensed physician within the last year and is in sound physical condition for participation in Ohio Players Tournaments. I authorize the staff of Ohio Players Tournaments & SW Ohio Basketball to act for me to their best judgement in situations requiring first aid or medical attention. I understand participation in athletics has certain inherent risks and assume responsibility for those risks. By registering for this tournament, I confirm that I understand that payments are non-refundable, regardless of prior notice of cancellation or the reason for cancellation. I further waive and release Ohio Players Tournaments & SW Ohio Basketball as well as staff, employees, contractors & volunteers along with those directing these tournaments from any and all liability for any injury my child might incur while participating in any and all Ohio Players & SW Ohio Basketball Tournaments.
