Good Morning All,
Please be sure that all players families fill out this ONLINE CONSENT! Rosters are based off of these consents, so using any player that does not have a completed online consent could result in a forfeit.
Families will receive an email after completing the consent to confirm. We will not be posting, or fielding questions on who on your team has completed the consents. If you want to track your team, ask the families to forward you the confirmation email they receive.

**SCHEDULES WILL BE POSTED BY 11:59PM TUESDAY, JULY 4**
ONLINE CONSENTS MUST BE FILLED OUT BY ALL PARTICIPANTS TO BE ELIGIBLE TO PLAY.
*Any player used that has not filled out a consent may result in the team being disqualified!
ENTRY FEE: $395.00 per team, 4 Game Guarantee
The 17th Annual Jimmy V Classic will be taking place at the Community First Champion Center in Appleton, Wisconsin.
The state-of-the-art facility features 8 wood basket courts, locker rooms and 2nd level balcony for all spectators.
This is an event to benefit The V Foundation
