IMPORTANT SCHEDULE REQUESTS POLICY
Due to the large number of special scheduling requests received for each event NCSTV charges a $20 fee PER TEAM for special requests. If you need a special time or accommodation request please include in the SCHEDULING REQUEST Section above. We will then GUARANTEE your REASONABLE requests will be filled or you will receive your $20 fee back. If you select a Scheduling Request without paying for it your request is NOT guaranteed. First come first serve on special requests. If we cannot meet your scheduling request your $20 will be refunded and we will communicate to you in advance. You will still be registered and scheduled for the tournament. (Each special request is $20, for example if you want to play in the morning and you can't play at the same time as another team due to having the same coach that is 2 requests).

AS THE COACH OR MANAGER OF THIS TEAM I AM TAKING RESPONSIBILITY FOR MY STAFF, PLAYERS AND FANS. I UNDERSTAND THAT I AM EXPECTED TO ADDRESS MY STAFF, PLAYERS AND FANS IF EVENT STAFF OR OFFICIALS ASK ME TO. IF I REFUSE TO ADDRESS MY STAFF, PLAYERS OR FANS I UNDERSTAND THAT MY TEAM MAY HAVE TO FORFEIT GAMES WITHOUT REFUND.
I am acquainted with the various risks of our team(s) participating in this activity including, without limitation, the significant risk of serious personal injury or death to myself and others and the significant risk of personal property damage or destruction, and hereby assume all risks and consequences associated with or arising in connection with such participation. I hereby agree to indemnify and hold harmless NorCal Sports TV, their employees, volunteers, and all organizations involved in the coordination, hosting, staffing and contribution of equipment, supplies, their agents, officials and employees from and against any and claims, damages, actions, liability and expenses in connection with loss of life, personal injury and/or damage to property arising from my participation in the above referenced activity. I hereby give NorCal Sports TV, its assigns, license, and legal representatives the irrevocable right to use my name, picture, portrait or photograph in all forms and media and in all manners, including composite, for advertising, for publication or any other lawful purposes, and I waive any rights to inspect or approve the finished product, including written copy, which may be created in connection therewith.
I hereby understand that NorCal Sports TV & its partners in the event will enforce a strict ZERO TOLERANCE for VIOLENCE Rules and Regulations. I hereby understand that violence or actions that lead to violence will result in my arrest and may also result in a lawsuit for my negligence. I hereby understand that NorCal Sports TV will turn over all information to support and cooperate with the law enforcement if there should be any charges be pressed against me.
I hereby agree to read the guidelines provided, by NorCal Sports TV & its partners in this event and it is my responsibility to understand the Code of Conduct and the Rules and Regulations.
I HAVE READ AND UNDERSTAND THIS WAIVER AND CONDUCT CONTRACT, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE DONE SO FREELY AND VOLUNTARILY WITHOUT ANY ASSURANCE OR GUARANTEE BEING MADE TO ME AND I INTEND THIS WAIVER TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
