2024 Liability Release Form In consideration of being allowed participating in any way in the Tournament of Champions 2024, I the undersigned: (A) Agree that the parent(s) and/or Guardian have been notified to instruct the participant that prior to participating, he/she should inspect the facilities and equipment to be used and if the participant believes anything to be unsafe, he/she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate. (B) Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death and severe social and economic losses which might result not only from their own actions, inaction’s or negligence of others, the rules of play, or the conditions of the premises or any equipment used. Furthermore, that there may be risks not known to us or not reasonably foreseeable at this time. (C) Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death. (D) Release the Hooptournaments.net, North GA Sting Basketball, Chris Tipton, Murray County Schools, Murray County or Whitfield Recreation Dept. and their respective administrators, directors, agents, coaches and other employees and volunteers, and if applicable, owners and the premises to be used to conduct the event, all of which are hereinafter referred to as releases from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, causes or alleged to be cause in whole or alleged to have been caused in part by negligence of the releases or otherwise. COACHES OATH I have full explained to the participants and their parents or guardians the risk involved in playing in the Tournament of Champions 2024, and I/we fully understand and we give up substantial rights by signing/agreeing this agreement.
