I realize that I am responsible for my own and my teammates' conduct during play and at the Tournament. I vow to behave in a sportsmanlike manner. In the event that I fail to do so, I understand that both myself and my team may be ejected from the Tournament without the possibility of any entry fee refund.
I acknowledge that participating in strenuous activities like basketball carries inherent risks. I assume all risks associated with playing in this event including, but not limited to: Falls, Contact with other participants, Conditions and hazards associated with any of the playing surfaces, Contact with the basket, basket support, bleachers, and/or any other structures, Exposure to extreme heat or other weather-related challenges
I understand that participation in any public event may carry health risks, including potential exposure to contagious illnesses. I choose to participate voluntarily and assume full responsibility for any such risks to my health.
I FURTHER AGREE TO RELEASE AND HOLD HARMLESS THE RELEASED PARTIES WHO, THROUGH NEGLIGENCE OR OTHER CONDUCT, MIGHT OTHERWISE BE LIABLE TO ME, MY PERSONAL REPRESENTATIVES, HEIRS, NEXT-OF-KIN, ASSIGNS, AND INSURERS FOR DAMAGES.
This Release, Waiver and Covenant Not to Sue extends to all claims of every kind or nature, foreseen or unforeseen, known or unknown. I hereby grant full permission to Summer Jam to use photographs, videotapes, motion pictures, recordings, or audio tapes of me or my likeness from this event for any purpose.
I EXPRESSLY AGREE THAT THE FOREGOING RELEASE, WAIVER AND COVENANT NOT TO SUE IS INTENDED TO BE AS BROAD AND INCLUSIVE AS PERMITTED BY THE LAWS OF THE STATE OF OREGON. IF ANY PORTION IS HELD INVALID, IT IS AGREED THAT THE REMAINDER SHALL CONTINUE IN FULL LEGAL FORCE AND EFFECT.
This document embodies the entire agreement with respect to the matters set forth herein, superseding any previous negotiations or understandings. I AGREE THAT NO PROMISE OR INDUCEMENT HAS BEEN MADE OTHER THAN WHAT IS EXPRESSLY STATED HEREIN. I UNDERSTAND AND VOLUNTARILY AGREE TO ALL TERMS. I HAVE BEEN ADVISED TO SEEK LEGAL COUNSEL PRIOR TO SIGNING.
Knowing these facts, and in consideration of you accepting my entry, I hereby, for myself, my heirs, executors, administrators, or anyone else who might bring claims on my behalf, covenant not to sue, and waive, release and discharge the Summer Jam 3x3 Basketball Tournament, all organizations involved with this event, the City of Springfield, Lane County, the State of Oregon, event organizers and entities, Summer Jam staff, volunteers, board of directors, committee members, and any and all sponsors of the event. This also includes their respective agents, employees, assignees, or anyone acting on their behalf from any and all claims of liability for death, personal injury, or property damage of any kind or nature arising out of or in the course of participating in this event.
Privacy Policy: At Summer Jam, we value your privacy and are committed to protecting your personal information. Any data collected during registration (such as names, contact information, and age) is used solely for the purpose of organizing and operating the event.
We do not sell, trade, or share your personal information with third parties, except as necessary for tournament logistics, communication, or as required by law.
By participating in Summer Jam, you consent to the use of photographs or videos taken at the event for promotional purposes on our website, social media, or printed materials.
