Waiver and Release of Liability
By completing registration for the AB1 Elite Basketball Camp, hosted by Locked In Sports in New Haven, CT, you acknowledge and agree to the following terms:
1. Assumption of Risk
I understand and acknowledge that participation in basketball training and athletic activities involves inherent risks of injury, including but not limited to sprains, fractures, concussions, or other serious bodily harm. I voluntarily assume all risks associated with participation in the AB1 Elite Basketball Camp, whether occurring before, during, or after camp activities.
2. Release of Liability
I, the participant (or parent/legal guardian of the participant if under 18), hereby release and hold harmless Locked In Sports, its owners, affiliates, directors, staff, coaches, volunteers, and facility partners from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may be sustained while participating in the AB1 Elite Basketball Camp, including travel to and from the event.
3. Medical Authorization
I authorize the camp staff to act on my behalf in the event of a medical emergency. I understand that I am responsible for any and all medical expenses incurred as a result of participation in the camp.
4. Personal Property
I understand that Locked In Sports is not responsible for any lost, stolen, or damaged personal property belonging to participants or attendees.
5. Code of Conduct – No Fighting Policy
I understand that all campers are expected to demonstrate respect, sportsmanship, and self-control at all times. Fighting, physical altercations, or aggressive behavior of any kind will result in immediate ejection from the camp without refund. By registering, I (and/or my child) agree to uphold this standard of behavior and understand the consequences of violating it.
6. Acknowledgment and Consent
By signing below (or checking the agreement box during online registration), I certify that I have read this waiver and fully understand its terms. I understand that I am giving up substantial rights by agreeing to this document, and I do so voluntarily.
