WAR Fall Camp Series - Registration - Aug 15

By registering for a W.A.R. Athletics sponsored event and accepting these terms, I assume responsibility for / of . . . PLAYERS CODE OF CONDUCT . . . for their conduct during the event. In the event that the behavior of the registered participant is deemed unsportsmanlike in any manner, the event reserves the right to disqualify and/or eject said individual for the remainder of the event in which they are prevented from re-entering the premises. Any fees or payments will be forfeited by the individual. - I will attend training on time, which means to arrive at least 5 minutes before the start of practice. - I will dress appropriately for training, bring enough bottles of water, a towel and extra clothes to change as needed. - I will follow the rules of the game and training, will be a good sport and demonstrate leadership in integrity. - I will give 100% effort, will not cut corners, will not give up on myself, my teammates or my coach. - I will control my temper and emotions. I acknowledge that fighting, “mouthing-off”, and cursing is strictly prohibited. - I will accept responsibility for my own performance and behavior. - I will treat everyone with respect regardless of race, sex, social status, disability, religion, identity, or skill level. - I will play for the enjoyment of the game. - I will respect the use of training aids and facility equipment. ACCIDENT AND RISK . . . of any risk my child may incur from participating in activities of W.A.R. Athletics basketball. I certify that my child is physically fit for participation in these types of activities and has not been advised not to participate by any qualified medical professional. I certify that there are no health-related reasons or problems which preclude my child’s participation in this activity. I hereby consent to for my child to receive medical treatment in the event of an emergency which may be deemed advisable in the event of an injury, accident, and/or illness during this activity. PHOTOGRAPHS, NAME, AND LIKENESS I understand while participating in the is activity, my child may be photographed. I agree to allow his/her photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns
