NOTICE: By signing this waiver, you acknowledge the inherent risks involved in attending youth basketball events, including but not limited to accidents, injuries, or damages. You voluntarily agree to waive any claims against the event organizers, facility owners, coaches, and any other affiliated parties.
1. Acknowledgment of Risk
I, the undersigned, acknowledge that attending a youth basketball event involves certain risks, including but not limited to accidents, injuries, and incidents that may occur on or off the playing court. I understand that the event organizers and affiliated parties will not be responsible for any injuries or damages that may occur during the event.
2. Release of Liability
I, on behalf of myself (and my minor child, if applicable), hereby release, waive, discharge, and covenant not to sue the event organizers, coaches, facility owners, volunteers, and all other individuals or organizations connected to the event from any and all claims, damages, or liabilities arising from my participation as a spectator.
3. Indemnification
I agree to indemnify and hold harmless the event organizers, coaches, volunteers, and other associated parties from any claims, damages, injuries, or losses resulting from my actions or those of my minor child.
4. Emergency Medical Care
In the event of an emergency, I grant permission for event personnel to provide or seek medical treatment for myself or my minor child. I understand that I will be responsible for any costs related to such treatment.
5. Media Release (Optional)
I grant permission for photographs or videos of myself or my child to be taken during the event for promotional, educational, or other purposes related to the event. I understand that these images may be used in various forms of media without compensation.
6. Acknowledgment of Understanding
I have read and fully understand the terms of this waiver and release of liability. I voluntarily agree to all conditions stated above.
