Memorial Day Classic - Registration

I give my team registered above permission to participate in the ThreatHoops event and hereby waive, release and forever discharge any and all claims against the organizers, sponsors, organizations or persons providing facilities, volunteers or any agent, and affiliate or employee thereof for damages and/or injuries to the participate which may arise from the participation in the ThreatHoops event. In addition, I hereby agree to indemnify and hold harmless the above described of and from all loss, damage and expense by reason of any claim for such damages and/or injuries brought or recovered by my team registered above against the above described. I understand that photos and videos will be taken at this event. I have obtained permission, from parent(s)/guardian(s) of the players representing my team, for ThreatCreative, LLC to use, reproduce, exhibit the likeness of the players in a photograph, video, or other digital media in any and all of its publications, including web-based publications, advertisements, articles and posts without the promise of payment, compensation or other consideration.
