LIABILITY WAIVER, RELEASE, AND ASSUMPTION OF RISK AGREEMENT**
This Waiver and Release of Liability (“Agreement”) is executed by the undersigned participant and/or the parent/legal guardian of the participant (if under 18 years of age) in favor of Locked In Sports, LLC, Locked In Basketball, their owners, staff, volunteers, contractors, sponsors, affiliates, facility partners, and event partners (collectively referred to as “Released Parties”).
1. ASSUMPTION OF RISK
I understand and acknowledge that participation in basketball activities, tournaments, camps, showcases, and related events involves inherent risks, including but not limited to:
Physical injury (sprains, fractures, paralysis, or death)
Collisions with other players
Equipment-related injuries
Facility-related hazards
Heat exhaustion or dehydration
I knowingly and voluntarily assume all risks, both known and unknown, associated with participation in any Locked In Sports event.
2. RELEASE OF LIABILITY
I hereby waive, release, discharge, and hold harmless the Released Parties from any and all claims, demands, damages, losses, costs, or expenses arising out of or related to participation in the event, including those caused by negligence, to the fullest extent permitted by law.
This release applies to:
Personal injury
Property damage
Medical expenses
Loss of income
Any other damages arising from participation
3. MEDICAL CONSENT
In the event of a medical emergency, I authorize Locked In Sports and its representatives to seek emergency medical treatment for the participant. I understand that:
Any medical costs are my sole responsibility
Locked In Sports is not responsible for medical expenses
I am responsible for providing proper health insurance coverage
4. CODE OF CONDUCT & EJECTION POLICY
I acknowledge and agree that players, coaches, parents, and spectators are expected to conduct themselves in a respectful and sportsmanlike manner at all times.
I understand that:
Unsportsmanlike behavior, fighting, or abusive conduct may result in immediate removal from the facility
Teams or individuals may be disqualified without refund
Locked In Sports has full authority to enforce event rules for safety and professionalism
5. PHOTO & VIDEO RELEASE
I grant permission for Locked In Sports and its partners to use photographs, videos, and recorded media of the participant for:
Marketing and promotional materials
Social media content
Website usage
Broadcast or highlight footage
I waive any right to compensation for such use.
6. REFUND POLICY
I understand that all entry fees, registration fees, and admission fees are non-refundable, except in cases where the event is canceled by Locked In Sports. No refunds will be issued for team disqualification, forfeiture, or participant withdrawal.
7. MINOR ACKNOWLEDGMENT
If the participant is under the age of 18, the parent or legal guardian acknowledges full responsibility for the minor’s participation and agrees to all terms of this Agreement on their behalf.
8. SEVERABILITY & JURISDICTION
If any portion of this Agreement is found to be invalid, the remaining portions shall remain in full force and effect. This Agreement shall be governed by the laws of the state in which the event is held.
9. ACKNOWLEDGMENT OF UNDERSTANDING
I have read this Waiver and Release of Liability fully and understand its contents. I am signing this freely and voluntarily and acknowledge that this Agreement is legally binding.
