Zoom Sportz Summer Basketball Camp Liability Waiver Policy
Introduction: Welcome to the Zoom Sportz Summer Basketball Camp! Our goal is to provide a fun and safe environment for young athletes to develop their basketball skills. By registering for our camp, you are agreeing to the terms outlined in this Liability Waiver Policy. Please carefully read the following information before signing.
1. Acknowledgment of Risk: I, the undersigned parent/guardian, acknowledge that participation in the Zoom Sportz Summer Basketball Camp involves physical activity that may expose my child to various risks, including but not limited to accidents, injury, or illness. I understand that basketball and related activities may involve risks such as falling, colliding with others, and other unforeseen accidents.
By signing this waiver, I accept and understand the inherent risks associated with these activities, and I agree to assume full responsibility for any injuries or damages that may occur during my child’s participation in the camp.
2. Medical Authorization: In the event of an emergency, I grant the camp staff permission to seek medical treatment for my child if necessary. I acknowledge that I am responsible for ensuring that my child is in good health and physically able to participate in the camp activities. I also agree to notify the camp organizers of any medical conditions, allergies, or special needs that may require accommodation.
3. Release of Liability: I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless Zoom Sportz, its employees, agents, directors, and volunteers from any and all claims, demands, actions, or causes of action arising out of or in connection with any injury or damages sustained by my child during their participation in the Zoom Sportz Summer Basketball Camp, including any injury or damage resulting from the negligence of the camp organizers, employees, agents, or other participants.
4. Indemnification: I agree to indemnify and hold harmless Zoom Sportz, its employees, agents, directors, and volunteers from any and all claims, demands, damages, costs, or expenses, including attorney's fees, that may arise from my child’s participation in the Zoom Sportz Summer Basketball Camp or from any actions taken by me, my child, or my representatives during the camp.
5. Permission for Photography and Video: I grant permission for my child to be photographed, recorded, or filmed during the Zoom Sportz Summer Basketball Camp. I understand that these images or videos may be used for promotional, marketing, or educational purposes without compensation or further consent.
6. Behavior and Code of Conduct: I agree that my child will adhere to the behavior guidelines and code of conduct set by the Zoom Sportz Summer Basketball Camp. I understand that failure to comply with these guidelines may result in my child being removed from camp activities without a refund.
7. Governing Law: This Liability Waiver shall be governed by and construed in accordance with the laws of [Your State], and any disputes arising hereunder shall be subject to the exclusive jurisdiction of the courts in [Your State].
