If you have any questions, please call Tom at 513-235-9007 or James at 513-289-6223 I certify that my son and/or daughter has had a physical examination by a licensed physician within the last year and is in sound physical condition for participation in Ohio Players Tournaments. I authorize the staff of Ohio Players Tournaments & SW Ohio Basketball to act for me to their best judgement in situations requiring first aid or medical attention. I understand participation in athletics has certain inherent risks and assume responsibility for those risks. By registering for this tournament, I confirm that I understand that payments are non-refundable, regardless of prior notice of cancellation or the reason for cancellation.
