Waiver and Release of Liability
By accepting these terms, you and all members of the entered basketball team hereby acknowledge and agree to the following terms and conditions as a prerequisite to your participation in the "Locked In Hoops Clinic" organized by Locked In Sports:
Acknowledgment of Risks:
I understand and acknowledge that participating in the "Locked In Hoops Clinic" involves certain risks and dangers, including but not limited to physical injury, property damage, or other losses that may arise from the actions or negligence of other participants, coaches, spectators, or conditions of the facilities where the clinic takes place.
Waiver of Liability:
By participating in the "Locked In Hoops Clinic," I, on behalf of myself and all team members, hereby release, waive, and discharge Locked In Sports, its officers, employees, agents, representatives, and any affiliated parties from any and all claims, liabilities, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by any member of the registering basketball team while attending or participating in the clinic.
Indemnification:
I agree to indemnify and hold harmless Locked In Sports, its officers, employees, agents, representatives, and any affiliated parties from any claims or liabilities asserted against them as a result of any injuries, damages, or losses incurred by any member of the registering basketball team during the "Locked In Hoops Clinic."
Assumption of Risk:
I acknowledge that participation in the "Locked In Hoops Clinic" is voluntary, and I assume all risks and responsibilities associated with our participation, including but not limited to transportation to and from the event, and while on the clinic premises.
Compliance with Rules:
I agree that all team members will abide by the rules, regulations, and instructions provided by Locked In Sports and the officials during the "Locked In Hoops Clinic."
Emergency Medical Treatment:
In the event of an injury or medical emergency, I authorize the clinic organizers, their representatives, or any medical personnel to seek and administer necessary medical treatment for any team member.
Binding Agreement:
By participating in the "Locked In Hoops Clinic," I acknowledge that I have read and fully understand the contents of this waiver and release of liability, and I am bound by its terms and conditions.
